In 1987, some 15,000 hospitalizations and 12,000 deaths due unfavorable drug reactions (ADRs) were conveyed to the Food and Drug Administration and several cases went unreported (Hepler, 1495).Drug-related mortality and morbidity are normally preventable, and medicinal services can minimize the space of hospital stays, the number of ADRs, and the cost of care.
